Of course, getting to and from the doctor’s office may be an issue in and of itself. This is especially true if you or a loved one has a physical or behavioral health condition that leaves you homebound. Ignoring the care you need is not an option. • Evaluation of safety and environment in the home and support and evaluation for both patients and family members 

As a Redeemer Health Physician Practice, Redeemer Health House Calls has access to a comprehensive range of services for older adults. Should a patient require hospital services, the House Calls team will make arrangements for the patient’s care.

Prescriptions

For prescription refills, please submit your request through the Patient Portal, or call 215-856-1100. Please allow at least 72 hours for the referral to be completed. You'll need the following information:

• Your full name
• Your date of birth
• Your phone number
• Name of medication, dosage (30 or 90-day supply)
• Pharmacy name and number

Services

Redeemer Health House Calls provides ongoing medical care to patients of all ages who are homebound and can't get to the doctor's office because of their advanced age, disability, or other qualifying socioeconomic situation. In addition to providing clinical care, we spend a great deal of time educating loved ones and caregivers on how to care for and manage the patient’s individual condition, as well as recommend community programs and services that may be helpful.
